Hyperthyroidism (Overactive Thyroid)

Evidence-based clinical insights and management strategies by Dr. Ibrar Ahmad.

📌

Key Facts

Hyperthyroidism occurs when your thyroid gland produces too much of the hormone thyroxine (T4). Hyperthyroidism can accelerate your body's metabolism, causing unintentional weight loss and a rapid or irregular heartbeat.

🧬

Pathophysiology

Excess thyroid hormone increases the metabolic rate and sensitivity of the sympathetic nervous system. Common causes include Graves' disease, toxic multinodular goiter, or a toxic adenoma.

🌡️

Symptoms & Clinical Features

Sudden weight loss, even when your appetite and food intake remain the same.
Rapid heartbeat (tachycardia) and palpitations.
Increased appetite.
Nervousness, anxiety, and irritability.
Tremor — usually a fine trembling in the hands and fingers.
Sweating and increased sensitivity to heat.
Changes in menstrual patterns.

Diagnosis

Diagnosis is confirmed with low TSH and high Free T4/T3 levels. A radioactive iodine uptake test (RAIU) and thyroid scan help determine the cause.

📊

Diagnostic Pathway

1
Initial Screening

Symptom review and baseline hormonal blood tests.

2
Confirmatory Testing

Advanced diagnostic protocols (e.g., OGTT, MRI, Suppression tests).

3
Personalized Plan

Tailored treatment strategy based on clinical results.

Treatment & Management

Treatments include anti-thyroid medications (Methimazole, PTU), radioactive iodine therapy to shrink the gland, or surgery (thyroidectomy). Beta-blockers are often used to manage heart rate and tremors.
