Key Facts

Hypothyroidism is a common clinical syndrome resulting from a deficiency of thyroid hormones, which in turn leads to a generalized slowing of metabolic processes. It affects approximately 4-5% of the population, with a significantly higher prevalence in women and the elderly.

Pathophysiology

The most common cause globally is iodine deficiency, whereas in iodine-sufficient regions like Pakistan, Hashimoto's thyroiditis (autoimmune) is the primary cause. Primary hypothyroidism involves the destruction of the thyroid gland itself, leading to low T4 and a compensatory rise in TSH. Secondary hypothyroidism results from pituitary or hypothalamic dysfunction.

Symptoms & Clinical Features

Fatigue, lethargy, and slowed physical/mental activity.
Cold intolerance and decreased sweating.
Unintended weight loss and constipation.
Delayed relaxation of deep tendon reflexes.
Myxedema (non-pitting edema) and coarsening of skin/hair.
Bradycardia (slow heart rate).

Diagnosis

Serum TSH is the most sensitive screening test. Primary hypothyroidism is confirmed by an elevated TSH (>4.5 mIU/L) and low Free T4. Subclinical hypothyroidism is defined by an elevated TSH with a normal Free T4.

Treatment & Management

The mainstay of treatment is Levothyroxine (synthetic T4). Patients must take the medication on a strictly empty stomach, 30-60 minutes before breakfast, to ensure optimal absorption. Follow-up TSH monitoring is typically performed 6-8 weeks after any dose adjustment to achieve the euthyroid target (usually 0.4-2.5 mIU/L).
