A prolactinoma is a noncancerous tumor of the pituitary gland that produces too much of the hormone prolactin. High prolactin levels can decrease levels of sex hormones (estrogen and testosterone).
Pathophysiology
The tumor (lactotroph adenoma) secretes prolactin autonomously. Excess prolactin inhibits the secretion of GnRH from the hypothalamus, leading to low FSH and LH levels and subsequent hypogonadism.

Diagnosis
Diagnosis is based on serum prolactin levels (typically >200 ng/mL in large tumors). Other causes of high prolactin (hypothyroidism, medications) must be ruled out. A Pituitary MRI is used to visualize the tumor size (microadenoma vs macroadenoma).

Treatment & Management
Unlike other pituitary tumors, medical therapy is usually the first choice. Dopamine agonists (Cabergoline or Bromocriptine) are highly effective at shrinking the tumor and normalizing prolactin levels. Surgery is reserved for patients who don't tolerate or respond to medication.
